ABSTRACT

Van der Knaap disease or megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a rare autosomal recessive degenerative disorder characterized by megalocephaly, cerebral leukoencephalopathy, and motor deterioration. Most cases reported with this disease are from our country India, belong to Agarwal community, who have high rates of consanguinity. We report a 4 and 1/2year old boy, with a history of delayed motor milestones, ataxia, increasing head circumference and abnormal body movements, who is belonging to the Bhat family of Handwara town of Kupwara district of Jammu and Kashmir, India.

ABSTRACT

Objective:To assess the efficacy of local steroid injection in trigger finger


Materials and Methods:This prospective study was conducted at department of Orthopaedics,Civil Hospital, Dow University of Health Sciences, Karachi, from March 2012 to 2013. Males and females with complain of trigger finger were included in this study during the specified period. All patients were injected depomedrol with plain xylocaine at the nodule site and functional outcome was assessed with respect to pain relief and finger movement


Results: Study included 43 patients with trigger finger.29 were males and 14 were females. Right hand fingers were involved in 24 patients, remaining had left hand fingers involvement. 10 involved the index finger,7 middle finger, 4 ring finger,10 little finger and 12 thumb. 9 patients had associated rheumatoid arthritis and 15 were known cases of diabetes. Remaining patients were primary with no known co-morbid. All patients had pain relief initially and regained movement of finger. 10 patients required repeated injection after 3 months. Maximum pain relief and regaining of movement after steroid injection was 4 months


Conclusion:Local steroid injection provides excellent symptomatic pain relief and regained movements without any complication in patients with trigger finger

ABSTRACT

Objective: To assess the functional outcome of radial head fracture Mason type III and IV treated with K-wire fixation


Materials and Methods:This prospective study was conducted at Department of Orthopaedics,Civil Hospital,Dow University of Health Sciences Karachi from 2013 to 2015. Patients with Radial head fracture type III and IV were included in the study


Results:A total of 42 patients were included in this study. Out of these 36 were males and 6 were females. Right side was injured in 18 patients and left side in 24 patients. 32 patients were with Mason type III radial head fracture and 10 were Mason type IV fractures. All fractures were fixed with open reduction and internal fixation with K-wire. Lateral approach was used in all fractures. In Type IV elbow was reduced with close technique. Outcome was measured on the basis of Morrey elbow scoring system. 22 patients with type III radial head fracture had excellent results while10 with type III had good result. All patients with radial head fractureType IV had good results


Conclusion:Assessment of functional outcome of radial head fracture Mason type III treated with K-wire fixation showed excellent outcome in majority of patients while Type IV had good outcome.Mason type III radial head fractures should be reduced and fixed with K-wire. It maintains the stability of elbow joint and elbow function

ABSTRACT

Objectives: The study was undertaken to investigate whether lead toxicity can reduce the diameter and spermatogenesis of testes of albino rats. If the reduction occurs, with what dose the animals can be protected by vitamin C against lead toxicity. Study Design: Experimental study. Place and Duration of Study: This study was carried out at the Department of Anatomy, PGMI, Lahore from March 2007 to August, 2007. Materials and Methods: In this study, 90 animals [albino rats] were taken from National Health Institute Islamabad. These were divided into five groups. Each group has 18 animals as group A, B, C, D and E. Results: The lead treated animals reduced 16% of diameter in 4 weeks as reported by Harvey. The loss of diameter I and reduction in spermatogenesis was due to lead toxicity. In another study by Biswas and Gosh, the animals gave same results with lead toxicity in 14 days. In this experiment it has proved that lead toxicity reduced the body weight of albino rats and this toxicity can be protected with heavy dose of vitamin C. Conclusion: Vitamin C reduces the toxic effects of lead on diameter of seminiferous tubules and spermatogenesis of rats, which is shown in this study

ABSTRACT

Greco-Arab system of medicine is based on fundamentals postulated by Hippocrates [460-377 B.C.]. Besides other concepts, the humoural and temperamental [mizaj] theories are the back-bone of this system and a very important base-line of Tibb is the concept of Mizaj-e-Insani [human temperament] which revolves around the diagnosis and treatment modalities as well as specific constitution of an individual. But as modern mind always seeks for more information which can only be provided by research on modern lines with the support of methods which were not available to the ancients. Therefore a comparative study was conducted between different somatotypes and Mizaje Insani at Aligarh city of Uttar Pradesh, India. Subjects were adults of age 19-25 years. Human mizaj was assessed on the basis of proforma developed to reveal Ajnase Ashra [ten parameters] as given in Unani literature, whereas somatotypes were assessed on Carter and Heath ratings. Two Types of Mizaj were selected for study i.e. Damvi and Balghami and their morphology was compared with different somatotypes. Significant difference was found between Damvi and Balghami [Mizaj] of their endomorph and slightly insignificant was documented between Damvi and Balghami and it was found that that majority of Damvi were mesomorphs where as majority of Balghami were endomorphs, which is consistent with the concept of Unani System of Medicine i.e. people of Damvi mizaj are more muscular and robust whereas Balghami mizaj people have more fat deposition

ABSTRACT

Objective of the study is to evaluate the efficacy of intramuscular ephedrine along with preloading in prevention of post spinal hypotension in elderly patients undergoing inguinal hernia surgery. This is a quasi experimental study. The study was conducted at the department of Anaesthesia and Intensive Care Combined Military Hospital, Peshawar over a period of one year. In a double-blind, randomized study, 80 elderly patients undergoing inguinal hernia surgery under spinal anaesthesia divided into two equal groups of A and B. Forty patients received i/m inj of ephedrine 45mg deep in the paravertebral muscles immediately after injection of bupivacaine, and 40 received an equal volume of saline. Patients in both groups were given the same volumes of fluid before anaesthesia. The incidence of hypotension [Systolic arterial pressure <90mmHg or <80% of baseline] were recorded. and incidence of fall in the heart rate was recorded. Systolic arterial pressure during the first 60 min after anaesthesia remained significantly more stable in the ephedrine-treated group, and there was also a significantly smaller number of patients in this group who had decreases in pressure of more than 30% of pre-block levels and fewer required rescue i.v. Ephedrine. An increase in heart rate or systolic pressure of > 20% from baseline was found in two patients in the ephedrine group and in one patient in the placebo group. We conclude that ephedrine 45mg administered in the paravertebral muscles immediately after plain bupivacaine spinal anaesthesia is a simple and effective means of reducing the incidence of hypotensive episodes in the elderly patient

ABSTRACT

To evaluate the effects of intravenous immunoglobulin therapy on progression of severe sepsis in patients of poly trauma. Quasi-experimental study. Combined Military Hospital Peshawar from June 2008 to Dec 2009. Forty six patients of poly trauma with severe sepsis were included. Along with the standard management i.e., surgical management, fluid resuscitation, antibiotics, analgesics, ionotropic, ventilatory and nutritional support, IVIG 5% [intravenous immunoglobulin] was infused over a period of 6 hours and repeated for three consecutive day. Sequential Organ Failure Assessment [SOFA] score was used to assess the progress in all the patients. At the time of enrolment mean SOFA score was 5.41 +/- 1.127 and on the 15th day it was 1.62 +/- 2.24, mean age was 39.21 +/- 10.26 years. Thirty four patients [73.91%] developed gram negative sepsis and eighteen patients [39.13%] developed septic shock. Mean duration of stay in ICU and on of these patients was 30.43%. The IVIG administration, when used along with the standard management appears to improve significantly the prognosis in patients of poly trauma with severe sepsis
